The body of Gnathophyllum modestum is short and thick, the carapace has a moderate carina that continues in front of the rostrum and extends about 2/3 of the way to the posterior margin.
Its rostrum is obliquely blunted dorsally and has 5 - 6 dorsal teeth and 1 - 2 small ventral teeth at the tip.
The eyes are rather large and have a prominent conical black projection on the cornea.
The third maxillipeds have a broad merus and carpus closing the entire anterior surface of the buccal region.
The body is deep brown with many scattered small yellow spots and a few larger orange spots, the legs are brown proximally and purple to white distally.
Gnathophyllum modestum may be associated with Oculina corals or sponges growing on rocky ledges.
